You can update your personal information such as a change in your Residential Address, your Mobile Number or your Email Address by submitting this online form (CLICK HERE).
Note: You can also request a change of Name, but we’ll need you to provide a reason for the requested name change and attach ONE (1) proof of identity (e.g. utility bill, birth certificate, marriage certificate, divorce certificate, passport, Medicare card OR driver’s licence) to verify your change of name.
